Because the real value starts after that.

First signal
Who noticed it first
Who could act
What slowed the response
What broke in communication
What supervision added or removed
What actually needs to change

If you stop at blame
you get silence next time.

If you go deeper
you get better systems.

Real reviews don’t protect egos.

They protect future operations.
